Below are the links to download and print the English and translated versions of the MMS Handbook Acknowledgement form, if you are unable to use the Google Form link provided above.
This form needs to be turned in to the front office no later than the first day of school for your student in order for your student to be assigned a Chromebook. One form is required for each student registered at MMS in each household.
